With over 90,000 individuals living with diabetes in our community—and numbers expected to rise—diabetes education and care have never been more vital. At Trillium Health Partners, our Adult Diabetes Services is a multidisciplinary team that provides expert care and support to over 15,000 individuals each year through our Diabetes Management Centre (DMC) and Centre for Complex Diabetes Care (CCDC).
Centre for Complex Diabetes Care (CCDC)
The CCDC provides specialized, collaborative care for individuals living with complex diabetes care needs. Our inter-professional team works together to develop personalized management plans that address the unique challenges associated with more complicated diabetes conditions. Through a short-term case management model, we support patients in stabilizing their diabetes and improving overall health outcomes.
We serve individuals living with both Type 1 and Type 2 diabetes, with a focus on empowering patients to manage their condition effectively. Through individualized education, access to resources, and coordinated care, our goal is to help patients improve their health and quality of life. Position Summary:
- Responsible for development of nutrition care plans and counselling for complex patients with diabetes and multiple co-morbidities such as renal, cardiovascular disease and mental illness
- Using a variety of teaching methods, incorporating the principles of adult education and motivational interviewing within a case management model
- Broad understanding of nutrition resources available in the community to help support clients with complex needs
- Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
- Functions as part of a multi-disciplinary team and acts as a resource for other health care providers and in the community
- Collaborates in daily team case conferencing to develop an integrated care plan and prepare for discharge planning
- Delivers education programs at the CCDC and in community settings
- Develops nutrition education materials
- Participates in program planning and evaluation
- Proficiency in computer, experience with electronic medical record and good keyboarding skills are essential
- Note, due to the complexity of the patient population, and collaborative team based short-term case management model, this role is not available as a hybrid or remote work
Key Qualifications:
- Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
- Member in good standing with the College of Dietitians of Ontario
- Current membership in Dietitians of Canada and Diabetes Educator Section of Diabetes Canada preferred
- Certified Diabetes Educator an asset
- At least 5 years current experience in a health care centre, and teaching complex clients with Diabetes in a group and individual setting is essential
- Good working knowledge of diabetes and other comorbidities through the lifecycle and adult teaching principles
- Experience using medical directives an asset
- Individual counselling and group counselling experience preferred
- Demonstrated leadership and communication skills
- Positive work and attendance record essential
- Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment
- Ability to speak a second language an asset, (ie Arabic, Tagalog, Vietnamese, Portuguese)
